Please complete the Confidential Questionnaire below.  When completed, either e-mail or fax the form to us (our fax number and e-mail address are at the end of the form).  Please complete it to the best of your ability - if you are unclear about anything, feel free to leave that section blank.  This form must be sent to us before our Get Acquainted meeting.  The more information that you provide to us, the better we will be able to work with you.

If you have some of your account information in a different format and would prefer to fax it, our fax number is 412-774-2116.  Feel free to fax at any time of the day - the fax comes securely into our e-mail and your information will not be sitting on a fax machine.
